What's the best extension for playing music

Which method/extension is the most performance friendly for playing during gameplay?

It depends what you need the extension for.

AMCAF has a whole bunch of useful commands and is highly optimised.

Personally I think its worth the effort to use AMOS's MUSIC command (after converting from Sound/Protracker) because it is more controllable than (say) Track Play or Med  which just play regardless.

Have a look in the manual for the functionality aspects but as always It' subjective.
